Volunteer Information Form

The place to start as an OTP volunteer is the Volunteer Information Form. This tells us what you are interested in and how to contact you. You can fill it out digitally or print it. Send it via email (to office@oldtownplayhouse.com), postal mail (to OTP, PO Box 262, Traverse City, MI 49685), or drop it off at OTP. We will be in touch and send you our volunteer newsletter.
