Participation Agreement

Everyone that works on an OTP project, production, or event must have a signed Participation Agreement on file in the Box Office to participate in OTP activities. This ensures that OTP and each person understand the liability and media relationship participating establishes between us. It also gives OTP an opportunity to ensure that participants have accurate contact information in our system to assign complimentary tickets, acknowledge volunteer activities, and communicate. You may fill out a Participation Agreement using an online form, or download the form and print (please return paper forms to the Box Office) or fill digitally and email (to tickets@oldtownplayhouse.com).
